How to Find and Merge Duplicate Patients

If two charts were accidentally created for the same patient and you would like to merge them as one chart this article will show you how.

Before you start: How merging works

Merging patients will combine the two patient records into one. Any appointments and health history will be moved on the final patient. If there is conflicting information (e.g. email address, name, cell number) between the two patients, you will choose one of the values, and the other will be lost.

🚨 Merging patients cannot be undone, so be sure the two patient records are for the same person before you merge.

How to merge patients

  1. Login to your account at Xchart.com

  2. Navigate to the "patients" tab towards the upper left corner of your screen.

  3. Select "search for duplicates"

  4. This will bring up lists of potential duplicate patients, based on matching birthdates and similar names. These are grouped into categories.

Quick merges

Patients that are considered as "quick merges" will have exactly the same birthdate and name, and no conflicting information.

It is likely that these are all duplicates, you can merge them individually, or, after reviewing that they are all correct you can press "merge all" to have them all merged.

Merges with conflicts

After the quick merges, any potential duplicates with conflicting information will be listed.

These may include: name or name formatting differences, or contact conflicts. These should be reviewed carefully. Where there are conflicts, choose the value from either side that you wish to keep, then press "Merge".

🚨Be careful here. Just because patients have similar names, does not mean they are duplicates. They could be two different people, and merging cannot be undone.

When you press merge, you will be shown a preview of the resulting patient, and the information that will be discarded. To merge the patients, confirm by pressing "Merge patients".

Advanced: Conflicts with external app ids

This section only applies if Xchart is linked to an external EMR or other application where patient data is linked to external patient records.

If Xchart is linked with an external EMR, the source of the duplicate may be in the external EMR itself. This makes merging patients a two step process:

  1. You must first merge the patient in the external EMR or other application. And note which id was the one that was kept.

  2. Merge the patients inside Xchart, and select the id from step one as the conflicting final value.

Xchart will show you the potential patients. Note that they both have an "APP ID" ("APP" should reflect the name of your external EMR/application so may differ).

Clicking Resolve will give you further instructions.

Once you have merged the patient in the external application, and have noted the resulting "APP ID" choose it in the Xchart interface and "Merge patients".
