How to Chart Throat Pack Placement in Xchart

If you place a throat pack or other commonly used items during a procedure and would like to document the placement and removal times, this article will walk you through the process step by step.

🌟After you have opened the anesthesia record for a specific patient follow the below steps. 🌟

Recording and Removing Common Events (Example: Throat Pack)

  1. Select "Record Event" from the top of the patient's chart.

  2. Choose the common event you would like to track. For this example, select "Throat Pack Placed."

  3. Click "Save."

  4. Once the event is saved, you'll notice a new button at the top of the patient's chart labeled "Remove Throat Pack." When the throat pack is removed, click this button to automatically time stamp its removal.

  5. You can also record the removal from the Timeline section of the chart, where the event and its removal can be time stamped.

Important: The Timeline will remain incomplete until the recorded event has been marked as removed. This feature is designed to serve as a reminder that any item placed during a procedure—such as a throat pack—should also be documented as removed, helping to support patient safety and complete documentation.

6. If you are on the timeline page and you delete the removal event it will NOT delete the placement event. If you delete the placement event, it WILL also delete the removal event. You can still just hit edit and change the text if you want. So you can always control what it says.
